step one
Start the process by completing the appointment request below. The request includes providing your contact information, a behavior assessment and history form, and our COVID-19 and cancellation policies along with our training agreement. Complete the form as completely as you can. Take time to read our policies and agreements.
step two
Once you complete the form, you will be asked to pre-pay for your session in full. We use Square as our payment processor. Square works with any US-issued and most internationally-issued magstripe or chip cards bearing a Visa, MasterCard, American Express, Discover, JCB, or UnionPay logo. If you would like to use another payment service, like PayPal or Venmo, please contact us via email directly.
step three
Once you complete the appointment request, behavior history form, and submit your payment, you will receive an immediate email confirmation.
IMPORTANT: save your appointment confirmation email. We use Zoom for our video consultation, and your confirmation email will have a link to our “zoom room.”
